1- The Lab Tests Results Online Service (the “Service”) offered by the Al-Khalidi Medical Center (the “Center”) is intended to facilitate obtaining the Lab Tests Results (the “Results”) online. The Patient acknowledges and agrees that the Center and/or its officers are not responsible for anything that may result from the use of the Service whether in contract or otherwise in any way whatsoever.

2- The Patient acknowledges and agrees that the Results shall not constitute a medical advice from the Center in any way whatsoever, they are only results of the lab tests requested by the Patient and/or stated in the prescription provided to the Center by the Patient and is so requested by the Patient as per practice. The Service shall not substitute or minimize, in any way whatsoever, the necessity to obtain the advice of a specialized doctor who shall explain the Results to the Patient and provide the diagnosis, the medical advice and the remedy, if necessary, as per practice.

3- The Patient acknowledges and agrees that consulting a specialized doctor is necessary to review the Results in light of all the givings related to the Patients such as age, gender, medical history and the symptoms, in addition to other tests including, but is not limited to, radiology to provide the patient with an accurate medical diagnosis.

4- The Patient acknowledges and agrees that the Center shall not be responsible in case the Patient uses the Service and/or the Results in breach of these terms and conditions in any way whatsoever. The Patient acknowledges and agrees that it shall be fully and solely responsible in case of its breach of these terms and conditions. The Patient unconditionally and irrevocably indemnifies the Center for any responsibility / liability that may results therefrom.

5- The Patient acknowledges and agrees that it is fully aware that the internet is an international public network, thus use of the Service may entail various risks including, but not limited to, disclosure of the Results to third parties even though the Center uses security means and applications as the Center deems fit. The Patient acknowledges and agrees that the Center shall not be responsible for any risks, damages and/or consequences arising therefrom.

6- The Patient acknowledges and agrees that in light of the nature of the internet, the Center shall not be responsible in case of stoppage, disruption and/or inaccuracy of the Results for any reason whatsoever. The Patient agrees and undertakes to follow all necessary procedures to ensure correctness and accuracy of the Results including calling and/or visiting the Center.

7- The Patient acknowledges and agrees that the Center may use the Results and the information pertaining to the Patient including disclosure of same to any other party as the Center deems fit without any restriction or condition.

8- Use of the Service shall be at the Patients full responsibility and at its own cost. The Patient shall be fully responsible for the use of the Service, applications and/or devices that may cause risks to the Service or that my result in disclosure of the Results or any information pertaining to the Patient.

9- Use of the Service shall be through the website of the Center by using the password pertaining to the Patient for the Service.

10- The Patient acknowledges and agrees to be fully responsible for persevering the password from theft, loss or disclose to third parties and that it is fully responsible in case of breach of this obligation or misuse of the Service.

11- In the event of entering an incorrect password for two times, the Service will stop automatically and the Patient shall visit the Center to obtain the Results.

12- The Patient acknowledges and agrees that the use of the password is considered a use by the Patient. Therefore, the Patient shall be fully responsible of such use.

13-The Patient acknowledges and agrees that it requested and applied to the Service in its sole judgment and will and at its full responsibility without any responsibility on the Center for risks, damages and/or consequences arising from the Service whatsoever. The Patient acknowledges and agrees that is shall be fully responsible for any risks, damages and/or consequences arising from the Service and unconditionally and irrevocably indemnifies the Center from any responsibility arising from the Service.

14- These terms and conditions shall be subject to the laws of the Hashemite Kingdom of Jordan. 

I read and comprehended the terms and conditions above and agree to be bound by them without any reservation, condition or objection.
